HCC are seeking an Adults Social Worker for the 0-25 Team based in Apsley, Hemel Hempstead.
The team are working a mix from the office and home – visits are taking place face to face.

QSW with Adults experience. This team works with transition from childhood to adulthood, however this worker will be covering those cases seemed as "Adults".
Support people with disabilities to ensure they have a quality of life, work through safeguarding, complete connected lives assessments, work on duty rota, work in partnership with providers, and partner agencies i.e. health colleagues, Police and other departments in HCC.
