Lincolnshire County Council requires 2 full time and 1 part-time (18.5 hours) to join teams that cover the Lincoln area. The roles will predominantly be undertaken whilst working from home due to the Covid 19 pandemic but on occasion attendance to an office will be required.

Whilst the positions will predominantly cover the Lincoln area you may be asked to support and undertake work in other areas, but all within Lincolnshire county.

You will be responsible for providing statutory professional social work as part of a team supporting adults with care and support needs. You will demonstrate through practice experience a commitment to strengths-based practice and skills in supporting people to identify and build support networks

to achieve their identified outcomes. You will be responsible for assessment, care and support planning, implementing and reviewing packages of care and support on a varied and more

complex caseload. You will provide advice, support and mentoring to other Adult Care practitioners in relation to their cases. You will be expected to work with autonomy managing a

caseload and demonstrate a multi-agency approach integrating your practise with colleagues across the health and care sector.

Experience and a good understanding of the Mosaic system is vital.
